You will receive a copy of this form for your records. The forms is also sent to the League Age Group Secretary and to your Club Secretary.

I, as the Manager, have read the CUP RULES AND THE RESPECT CODES OF CONDUCT FOR CLUB OFFICIALS & COACHES and agree to abide by its content and understand that failing to do so could be severe for the registering club concerned. I confirm that the coaches and volunteers assisting with this age group have also read the appropriate codes of conduct and that they will abide by them.
